Print the form, fill it out and then fax to:

You will then be contacted by us (by phone and/or email) with a membership code and a 24-hour hotline number which will be your responsibility to check regularly, as jobs can change daily or even hourly. The hotline will prompt you exactly who to contact and/or exactly what you need to do to get booked on a job. You will be working for other agencies and booked directly through us. Once you are booked, the number one golden rule is to show up on any job you book yourself on, if you do not show up for a job without giving at least 72 hours notice you will receive a $10 penalty fee and may even be removed from the database--your membership terminated. We have a reputation to live up to and the professionalism of our members reflects Casting USA as a whole.
For regular extra work, sending in your photo is optional. For picture submissions and auditions, naturally you will want to provide us with your photograph. Send us a current photo of yourself with your full name and current contact telephone number, preferably both day and evening numbers.
